Becoming an EAP Provider
Due to the continual expansion of EFR`s EAP business throughout North America, the addition of providers to our affiliate network is an ongoing need. If you are interested in consideration for joining Employee & Family Resources` EAP provider panel, please review the criteria below. If you have questions about the criteria identified feel free to contact the Affiliate Network staff with any questions.
- Minimum of a master`s degree in the behavioral sciences.
- Possession of the appropriate state or provincial licensure, certification or registration or meeting the supervision requirements in preparation for obtaining such.
- Professional liability insurance (minimum of $1,000,000/$3,000,000).
- Annual participation in professional development activities which are to include addictions, crisis intervention, and professional ethics/clients.
- At least two of the following: training and experience in organizational dynamics; 2,500 hours of post-graduate clinical experience or regular supervision with a clinician who meets this criterion; CEAP designation; two years EAP experience; or master`s level internship in an EAP setting.
- A respected role in the community with a record free of license refusal or suspension, malpractice, or other judgments of professional misconduct; performance in a professional environment free of harassment, including sexual harassment, in respect to colleagues and clientele.
- Adherence to all applicable state and federal confidentiality guidelines.
- Compliance with federal and state law requirements regarding mandatory reporting of child and dependent adult abuse.
- Adherence to a non-discriminatory practice in which clients are not denied service based on race, creed, color, religion, political affiliation, national origin, gender, age, physical or mental disability, sexual orientation, or positive test for AIDS- related virus.
- Knowledge of community resources.
- Knowledge of substance abuse assessment sufficient to result in recommendations for any further assessment or treatment services by a qualified provider.
- Maintain 24-hour per day, seven days per week, crisis intervention or emergency service referral capabilities. In cases of emergency, have the capability of immediately meeting with the Covered Person or immediately referring the client to local emergency services.
If you believe you meet these criteria, you may download and return the "Request for Affiliate Credentialing Packet" form. Upon receipt of this form, we will notify you if we are in need of adding providers to our network in your area. If we do not have current need we will add your information to our "Potential Provider" database for future reference.
