Become an EAP Provider
Thank you for your interest in EFR's EAP provider network! To be considered for participation on our network, please review the eligibility requirements and if you feel this is a good fit for you, complete the Prospective Providerform. If we do not have current need in your area, we will prioritize your request when we are in need. If we are expanding the network near your location, we will contact you directly. Eligibility requirements
- Minimum of a Master's degree in any behavioral health field
- Licensure and/or certification in the state in which he or she practices
- Current malpractice insurance with minimum of $1 million/$3 million
- EAP experience and/or knowledge of EAP services
- Knowledge of community resources
Additional specialties preferred
- Certified Employee Assistance Professional (CEAP)
- Qualified Substance Abuse Professional (SAP)
- Training and/or certification in addictions assessment and referral
- Training and/or certification in CISD or trauma response
- Experience delivering workplace training and orientation
